More About Supported Living

Our supported living services are designed for adults who are ready to take the next step towards greater independence but continue to benefit from flexible support with everyday living. Working in partnership with each tenant, our teams help develop practical life skills, promote wellbeing, encourage community participation and support people to achieve their personal goals while maintaining choice and control over their lives.

Supported living often represents an important stage within IHCM’s integrated care pathways, providing a seamless transition from hospital, rehabilitation or residential services into independent living. Equally, many people are referred directly from the community or by local authorities where supported living is the most appropriate option.

We currently provide supported living services across North Yorkshire, the West Midlands, East Midlands, Berkshire, Essex, Gloucestershire, Devon and Cornwall, and continue to work with partners and commissioners to develop new services where there is local need.

Because these properties are people’s homes, we do not publish individual addresses or detailed locations. This helps protect the privacy, dignity and safety of tenants while ensuring they can enjoy their homes as members of their local communities.

What is supported living?

IHCM’s providers of supported living enable adults to live in their own home with flexible, person-centred support that promotes independence, choice and control. Unlike residential care, people living in supported living have their own tenancy or licence agreement and are supported to live as independently as possible within their local community.

Support is tailored to each tenant’s individual needs, goals and aspirations, helping people develop practical life skills, maintain their wellbeing, build confidence and participate fully in everyday life. The level of support is flexible and designed to encourage independence while ensuring people receive the assistance they need to live safely and successfully within their own home.

Who is supported living suitable for?

IHCM’s providers of supported living support adults who are able to live independently with the right level of personalised support. Our services are suitable for people with mental health conditions, learning disabilities, autism, acquired brain injury and other complex support needs, as well as individuals who are recovering from addiction and are ready to rebuild their lives within a safe, stable and supportive community environment.

Every tenant has their own home and receives a personalised support plan developed around their strengths, aspirations and individual goals. Our teams work alongside tenants to develop practical daily living skills, maintain their wellbeing, build confidence, access education, employment or volunteering opportunities where appropriate, establish positive social networks and participate fully in community life.

As part of IHCM’s integrated care pathways, supported living can provide the next step following hospital treatment, mental health rehabilitation, residential care, addiction treatment or other specialist services, helping people sustain their recovery while receiving the right level of ongoing support. We work closely with local authorities, NHS partners and other professionals to ensure every tenancy promotes independence, choice and long-term positive outcomes.

Why don’t you publish supported living addresses?

Supported living properties are people’s homes, not care facilities. To protect the privacy, dignity and safety of our tenants, IHCM and its providers do not publish individual property addresses or detailed location information on our website.

We are happy to discuss the areas we cover, property availability and referral pathways directly with local authorities, Integrated Care Boards (ICBs), commissioners, social workers and other authorised professionals, ensuring suitable accommodation is identified while respecting each tenant’s right to privacy.

Can support change over time?

Yes. One of the key benefits of supported living is that support can be adapted as an individual’s needs change. Every tenant receives a personalised support plan that is regularly reviewed with them, ensuring they continue to receive the right level of assistance while promoting independence and positive outcomes.

Support may increase during periods when additional help is needed or reduce as tenants gain confidence, develop new skills and become more independent. Our providers work closely with tenants, local authorities, healthcare professionals and families, where appropriate, to ensure support remains flexible, responsive and centred around each individual’s goals.

How do I enquire about supported living?

IHCM’s central referrals team works closely with local authorities, Integrated Care Boards (ICBs), social workers, commissioners and healthcare professionals to identify the most appropriate supported living service within our network of providers.
