Whether supporting people with long-term physical health conditions, complex mental illness, dementia, acquired brain injury or other specialist needs, our focus is on promoting wellbeing, maintaining independence and enhancing quality of life.
As part of IHCM’s integrated care pathways, our care homes with nursing work closely with hospitals, rehabilitation services, residential care and supported living providers to ensure people receive the right care in the right setting.

IHCM’s providers of residential care with nursing support adults who require 24-hour nursing care alongside specialist support for complex physical health, mental health, neurological or behavioural needs. Our services also support people living with complex dementia, acquired brain injury, progressive neurological conditions, learning disabilities, autism, and profound and multiple learning disabilities (PMLD) where ongoing nursing care and clinical oversight are required.
Many of the people we support have multiple or co-existing needs, requiring coordinated care from experienced multidisciplinary teams. This may include individuals with complex medical conditions, challenging behaviours, physical disabilities or long-term health conditions that cannot be safely managed within a standard residential care setting.
Every placement is individually assessed to ensure the service can meet the person’s clinical, nursing, therapeutic and support needs. Through personalised care planning and IHCM’s integrated care pathways, we work with NHS commissioners, local authorities and healthcare professionals to deliver high-quality nursing care, maximise quality of life and, where appropriate, support progression to less intensive services.
IHCM’s providers of residential care and residential care with nursing both offer safe, supportive environments where people receive personalised care that promotes dignity, independence and quality of life. The key difference is the level of clinical support available.
Residential care provides accommodation, care and support with everyday living for people who do not require ongoing nursing care. Care teams support individuals with daily routines, wellbeing, social inclusion and maintaining as much independence as possible.
Residential care with nursing provides the same personalised care and support, together with 24-hour nursing care delivered by registered nurses. These services are designed for people with more complex physical health, mental health, neurological or behavioural needs who require regular clinical assessment, medication management, specialist nursing interventions and ongoing multidisciplinary oversight.
Through IHCM’s integrated care pathways, individuals can move between different levels of support as their needs change, ensuring they continue to receive the right care in the most appropriate setting. Every placement is individually assessed to ensure the service can safely meet the person’s clinical, nursing, therapeutic and personal care needs.
Yes. Many of IHCM’s providers accept NHS Continuing Healthcare (CHC) funded placements, as well as placements funded by Integrated Care Boards (ICBs), local authorities and other statutory organisations. We work closely with commissioners, care managers, social workers and healthcare professionals to ensure each placement is appropriate for the individual’s assessed clinical, nursing, therapeutic and support needs.
Every placement is individually assessed by the relevant provider to ensure the service can deliver the right level of care and support. Our central referrals team works in partnership with commissioners to identify the most appropriate provider, discuss funding arrangements and develop personalised care pathways that promote continuity of care, positive outcomes and, where appropriate, progression to greater independence.
Yes, where appropriate. Many of IHCM’s providers support residents to develop the skills, confidence and independence needed to progress to less intensive support. Through personalised care planning, residents are encouraged to maximise their independence, develop practical daily living skills and work towards their individual goals at a pace that is right for them.
As part of IHCM’s integrated care pathways, some residents may progress to supported living, return to their own home with community support or move to another service that better reflects their changing needs. For others, residential care may provide the most appropriate long-term home, and our focus is on maintaining quality of life, promoting wellbeing and delivering person-centred support.
Every transition is carefully planned in partnership with the resident, their family where appropriate, local authorities, NHS commissioners and community services to ensure continuity of care and the best possible outcomes.
Families and carers play an important role in supporting the wellbeing, recovery and quality of life of the people we support. Across IHCM’s network of providers, we are committed to building positive relationships with families and carers, recognising the valuable insight and support they can provide. Wherever appropriate, and with the resident’s consent, families and carers are encouraged to be involved in care planning, review meetings and ongoing communication with the service.
Each of IHCM’s providers develops its own relationships with families and carers through a range of initiatives, which may include family forums, newsletters, meetings, open days and regular updates. These opportunities help keep families informed, encourage meaningful involvement and provide opportunities to share ideas and experiences.
