Yes. Many of IHCM’s providers accept placements arranged by local authorities, as well as referrals from Integrated Care Boards (ICBs), Provider Collaboratives, NHS Continuing Healthcare teams where appropriate, and other statutory organisations. We also welcome private enquiries where suitable.
Our central referrals team works closely with commissioners, social workers, care managers and healthcare professionals to identify the most appropriate provider and develop personalised care pathways that meet each individual’s assessed needs.
Not always. Residential care can provide either short-term or long-term placements, depending on an individual’s needs and personal goals. For some people, residential care forms part of a wider recovery or rehabilitation pathway, providing the support needed before progressing to supported living, returning home or moving to a less intensive service.
For others, residential care provides a longer-term home where they can continue to receive personalised support in a safe, stable and welcoming environment. Every resident has a person-centred care plan that is developed in partnership with the individual, their family or carers where appropriate, and the wider care team. Care plans are based on each person’s assessed needs, strengths, preferences, goals and aspirations, ensuring support is tailored to what matters most to them.
Care plans are regularly reviewed to reflect changing needs, monitor progress and identify new opportunities to promote independence, wellbeing and community participation. Families and carers are encouraged to be involved throughout the planning process, with the resident’s consent, helping to ensure care remains coordinated, personalised and focused on achieving positive outcomes.
